Job details:

The day-to-day life of a Medical Management Specialist I in this role would be dynamic, split between remote/virtual tasks, field visits, and team collaboration , especially since it's a travel-required role in the Chicago area . Here's a breakdown of what a typical day might look like:

☀️ Morning: Remote Work & Planning

Review new and existing member cases in the EMR (could be Epic, Allscripts, Athenahealth, etc.).

Plan your outreach schedule : prioritize high-risk or newly discharged members.

Check voicemails, emails, and updates from team members or providers.

Attend a morning team huddle via Microsoft Teams or Zoom , reviewing caseloads, updates, or recent discharges needing follow-up.

📞 Mid-Morning to Early Afternoon: Outreach & Field Visits

Begin outreach calls to members to:

Perform assessments.

• Educate on medications, diagnoses, or upcoming appointments.
• Confirm post-discharge plans.

Travel to member homes, clinics, or facilities (depending on need).

• Conduct in-person assessments and education.
• Coordinate with providers, case managers, or social workers.

Document all interactions immediately in the EMR system —this could include progress notes, updated care plans, or social determinants of health (SDOH) needs.

🍴 Mid-Afternoon: Follow-Ups & Collaboration

• Call providers or specialists to verify or coordinate treatment plans.
• Assist in transitions of care , for example, ensuring a patient discharged from the hospital has DME (durable medical equipment), meds, follow-ups scheduled.
• Possibly attend care coordination meetings or interdisciplinary rounds (virtually or in person).
• Help identify community resources (housing, food, mental health) and connect members to them .

📝 Late Afternoon: Documentation & Reporting

• Finalize documentation in EMR systems for compliance.
• Enter data for regulatory reports or performance tracking.· Prepare PowerPoint or Excel summaries if working on reporting tasks.
• Respond to any outstanding tasks or messages.

· If applicable, participate in training or case review sessions . ⚙️ Key Tools You’ll Use:
