Role Description

• Coordinates patient care with the physician and all members of the interdisciplinary team through ongoing communication and appraisal of patient’s changing condition. Refers to appropriate team member for assistance as needed.
• Ensures the educational needs of the patient and family/caregiver are met in order to promote optimal care.
• Provides appropriate pain/symptom management and evaluates patient’s response to treatments/medications
• Other duties and responsibilities as assigned.

Qualifications

• Active RN License in Colorado
• In good standing with DORA
• RN or BSN Degree
• 2 years Hospice experience preferred
• Valid Driver's License and Reliable Vehicle
