Position Overview:

Seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with specialization in wound care . The nurse will perform wound assessments, manage care plans, provide education to staff, and ensure residents’ skin and wound health in compliance with facility policies and clinical standards.

Key Responsibilities:

• Perform wound assessments, reassessments, and management for all residents.
• Develop skin care programs to prevent skin breakdown.
• Implement policies for management of wounds, fistulae, tubes, ostomy care, and incontinence.
• Revise care plans based on resident response and treatment effectiveness.
• Assess and manage pain; educate residents and families on pain management.
• Serve as a resource to nursing staff; provide in-services on wound care, ostomy care, and incontinence.
• Communicate effectively with nursing staff, physicians, and interdisciplinary team members.
• Ensure accurate documentation according to current standards.
• Stay current with training, continuing education, and best practices in wound care.

Required Qualifications:

• Active LPN or RN license in Arizona.
• Certification as a CWOCN (Certified Wound, Ostomy, Continence Nurse) or CWCN (Certified Wound Care Nurse).
• Ability to obtain and maintain Arizona DPS Fingerprint Clearance Card .
• Current CPR/BLS Certification .
• Local candidates only – available for in-person interviews and start within 2 weeks of offer.

Preferred Qualifications:

• Experience in a skilled nursing facility or long-term care environment .
