At Tufts Medicine Care at Home , we believe every patient and family deserves compassionate, dignified, and expert end-of-life care. We are seeking a Nursing Manager to oversee our dedicated team at High Pointe House in Haverhill as well as hospice care across skilled nursing facility settings.

What You’ll Do

• Oversee day-to-day clinical operations at High Pointe House and in skilled nursing facilities.
• Lead, mentor, and support a team of hospice clinicians and staff, ensuring adherence to best practices and standards of care.
• Monitor patient census, trends, and outcomes—using data to continuously improve care delivery.
• Collaborate with physicians, referral partners, and internal teams to ensure patients are identified and supported through their hospice journey.
• Ensure compliance with hospice and palliative care regulations, policies, and quality standards.
• Build relationships with referral sources and community partners to expand awareness and access to hospice services.
• Provide ongoing education, coaching, and performance feedback to staff, fostering a culture of compassion, accountability, and teamwork.

What We’re Looking For

• Bachelor’s degree in Nursing (BSN) required
• Current Massachusetts RN license
• 5+ years in hospice, palliative, or complex care
• 2+ years of progressive leadership or management experience
• Strong team-building, communication, and problem-solving skills
• Passion for leading a team that delivers patient-centered, family-focused care
